Patient Care Concerns

 

We strive to provide the highest quality of care and safety to all Olin Health Center patients. If you have a concern related to quality of care or patient safety, please contact the office of the Olin Health Center Ombudsman. The Ombudsman is available to discuss problems or concerns and will work with you to help resolve the situation.

  • Submitting a patient Compliment and Complaint form. The forms are available at the white "Compliment, Complaint, Suggestion" boxes located throughout Olin.
  • Use our web form, Ask Olin, to send an email note. (Please keep in mind that the Internet is the least secure option for voicing a concern.)
